Coordinated Plan Tool Kit

In order to be eligible for Human Service funding (5310 and/or AMHT), agencies and their communities must have a current locally developed Coordinated Public Transit-Human Services Community Transportation Plan. Listed below are resources for creating and updating a coordinated plan.
