Completing a Traveler Information Kiosk (TIK) Permit Application Permit Application Request

To acquire a Tik Permit, you must first pass the Minimum Qualifications.

The Applicant Information section on the form should show the name of the entity requesting the permit and the mailing address for the entity.

1. Fill in the Proposed Tik Sign Location Information fields.

   All required fields are denoted with a red asterisk (*)

   • Select the down arrow next to the field for Region.

     

   • Select the region name based on the map provided.
   • Enter the proposed kiosk location in the Location of the Proposed TIK Installation (Include highway name, milepost, or any other pertinent information.) field.

     

   • Enter the speed limit for your location in the Speed Limit field.

2. Fill in the Business Information Area.

   All required fields are denoted with a red asterisk (*)

   • Enter the business license number in the Business License Number field.

3. Fill in the Kiosk Services Information.

   All required fields are denoted with a red asterisk (*)

   • Enter the services for the kiosk in the Please list the tourist-oriented activities, recreational and cultural interest areas, and general services to be included in the kiosk: fields.

     If more space is needed, please attach in a separate document.

4. Indicate how you will submit your attachments in the Attachment Information section.

   

   If the files are primarily electronic and you have them, then indicate that you will attach them; otherwise, indicate that you will mail them.

5. Select the Submit button to request the permit.

   -OR-

   Select the Cancel button to exit to the main menu without saving your information.

   -OR-

   Select the Reset button to erase your information and start again.
